Community Tissue Services provides autologous tissue storage/irradiation for hospitals with active contracts. When recovering autologous tissue, if you do not have a pre-assembled Community Tissue Services autologous tissue kit, please use the forms listed. If you have any questions regarding the autologous tissue program, please contact one of our Community Tissue Services centers.
